Round or rectangular box/cover?

Single receptacle usually means a rectangular cover and also it's in a raised home.
If so, replace all of it if you're not sure.

Ha! Same issue here. Covers for floor receptacles seem to be as individually unique as snowflakes.

Yeah, I remember them. Should have held on to the cover.
The receptacle, you might be able to use one from the new in wood set ups cause they are recessed in. The screw on brass cover, no idea. Go into and old building and help yourself.
