After being on the phone with the State for about 2 hours - dept. of insurance and OMV - I reach out to the OT.

The state employees swear that there is a section on the expresslane.org site where citizens can download the form to request liability insurance information from the state. They claim that the portion of the site has the form, instructions, and pricing ($15). However, I cannot find it anywhere on their site.

There are two Form R0613's on the site. The one you want is under "Drivers' Management Bureau."

I can see why you couldn't find it.

The OMV site has things labeled horribly, and the form itself does not say in plain English what the hell it is for. If it's intended to obtain insurance information on the other driver, just fricking say so.

I didn't think I had to expressly state it; I assumed that was easily implied.

Drunk rolled through the front of the property...drunk cited for no insurance, but the plate was issued only a few months ago...cops said that drunk might still have insurance and to check with the state.
