Yeah, with it being bilateral he will most likely need an MRI. Bilateral signs are more concerning. It's possible he has two bulging disks (one in each direction), but again the imaging is likely necessary due to the bilateral nature of it.

The ortho's Physician Assistant will take some x-rays, give you a script, and possibly a shot if you're in intense pain.
He'll send you to a physical therapist who'll do you the most good. Some electro-therapy, some exercises...
If it doesn't get better in a month they'll suggest the MRI. Most people get better in a few weeks.



Going through this now. Mine usually flares up on my left side but after standing and cooking for a few hours before the Bama game I got it on the right side worse than ever before. Went to the after hours clinic on Sunday and got a Demerol shot that did nothing more than knock me out for the day which I was OK with.
Went to the orthopedic on Monday morning and they found a collapsed disc and bone spur in my lower back.
Lots of meds, a steroid injection and a week and a half later and I'm able to get up and go to work but still in pain. He wants me to go to PT for a few weeks to see if that helps then MRI if not.
I'm not too optimistic on the outcome since I have done PT before when my left side hurt with little benefit. Now that I have had the issue on both sides, I'm not so sure its muscular.
I wish I could say it gets better but it seems every episode I have had is worse than before. This time was unbearable for a few days and has lasted longer than the others.

If you're commuting, be aware of how you sit..
too much pressure on your sciatic every day could numb it. You might try to find a seat cushion.. I recommend the Gelco Gseat Lite; and if you sit in an office chair all day be even more careful. Consider a standing desk.

Also don't assume it's a piriformis issue and try to massage it - you could possibly further damage sciatic nerve like I did.

There are stretches in yoga that can relieve the pain. I would go PT first.

This X a Billion. I was the same age - 29 - just started having debilitating back pain. Went to my doctor first who got me on a PT regimen. Did that for a while. Flared up all the time. Excruciating pain. Read tons of articles on inversion therapy and found a table for $100 at Academy and grabbed it. changed my life - it has been 7 years pain free. Took about 2 weeks of inverting before my spine completely decompressed and it was like freaking magic.

I know nothing about back pain - discs, sciatica, etc - I just know that dam table worked.

This!

Involves both sides and with no associated trauma.
Odds are very low of it being a herniated disk.

But the symptoms warrant a visit to your PCP straight away..

bucket seats sometimes position knees higher than hips. This throws all upper weight onto piriformis, through which the sciatic runs.

A cushion, like the Gseat Lite I mentioned, lowers (or at least spreads out) pressure on lower back and sciatic; and may help elevate hips above knees.

I commuted for a year and a half and developed "foot drop," which is an extreme example of sciatica. That cushion has helped significantly. The inversion table helped too as others have noted.

Also, don't sit on a wallet. Ever. It's taken me almost 5 years to recover, and I was once a decent athlete. Hope this helps.. don't snooze on it!
