Because he put his real name into Periscope and then they looked at the map feature and coordinated those two tings up. Poster also clearly has a landline or has his cell number posted all over the internet for the world to easily find too.

I'm guessing they linked his TD name with his pscope name or comments and searched his TD history, which for some foolish reason contained his ph# or email or something else connecting to ph#.
