The wife and I left Louisiana back in 2007. We have legally insured all vehicles since then. We also turned in all necessary documents to La OMV once our vehicles were insured and registered in the new state. Both my truck and the wife's car are now being targeted for over $1000 in fines and we have done nothing wrong.
Now dealing with the OMV is a nightmare. Their phones are busy all day. When you do finally get through, the person you speak to has little to no answers to any questions. They offer hardly any way out of the fines. And this is all complete and utter bullshite.

Try the form. Can't hurt. It is a least a note of protest of the fines. Phones are useless. I used certified mail.
Also remember if out of state, only the following five states do not participate in the Drivers License Compact
Georgia
Massachusetts
Michigan
Tennessee
Wisconsin
Louisiana may frick you even if you have escaped.
How would it look to an Engineering Licensing Board to know your LA License was revoked?

Where are these articles? I saw something about if the case wasn't strong during those months, they might not enforce. That's not the same as not enforcing them at all, though.
If you fill out the form and you didn't follow the law at the time, wouldn't you be admitting some sort of violation?

I faxed in a copy of my VA registration and titling(that showed I registered the same day I "cancelled" in LA) and a letter from insurer stating that I never had a lapse insurance.
Just got a letter in the mail(to my parent's house again...even though my fax included all my VA mailing info) clearing me of all the fees including late fees.

Mine cleared today.
I had the affidavit and submitted what documentation I had. I only had supporting documentation from the tow and storage (mine was a total from a wreck) but not the actual salvage receipt or such.
Worth a shot.
I would think the affidavit and some supporting insurance papers would clear a lot of people. Maybe just affidavit for older cases, one would hope.
I found email to be the best way to proceed and was finally cleared with scanned and emailed documentation, although I had sent it all in by certified mail earlier.
My last comments on the matter, but what a shame we have to spend the time, have our finances and credit threatened, have our professional licenses threatened, and even be told we we be unable to buy a fishing license due to inaccuracies at the OMV. Or I guess I was lucky enough to be in the 1/3 that actually did not owe a fine per Edmonson.
