It varies by state, I'm over 25 and don't have anything on my driving record and I have good credit and I pay around $77.

I can't remember but its high I'm a state vendor so I have to carry $1 mil liability, I just looked and I pay $155/ month for 3 vehicles. It looked like it just went up $5 per vehicle. That's why I said $75 though to over. I've been a USAA customer for like 15 years and also have great credit with no accident history.
I'm not saying the OP doesn't receive a little benefit from driving his company car, but he also pays $350/ month. I guess that's no different than owning a vehicle and being paid mileage. My only point was he isn't getting a fantastic benefit that I wouldn't leave a job over.

I pay $350 a month for two vehicles with high limits/ low deductible and that is cheap to me.
