If primary care docs will get adequately trained in office procedures, take care of their patients, keep overhead low, code appropriately, and do most of their own ancillaries(EKG,PFT's,X-ray,etc..)the sky is the limit. Not to mention adding midlevel providers to generate more income and act as referral sources for your derm, injections, allergy etc. But you're gonna work. That's true in any field though.

That's interesting cause the school and other docs try to make midlevel providers seem like the bogeyman, coming to take away all our jobs.
I prefer your approach, and I'm sure patients do too, but it seems like a lot of capital is needed to get that sort of practice running. How soon after residency is that feasible? I like primary care but I'm up to my eyeballs in loans.

Yes, and I can point you to thousands of people that spent a grueling 4-8 years beyond undergrad earning a PhD in some area of physical science ( average length of time to earn a physics PhD is now 6 years), then enduring several years of post grad appointments hoping to eventually land a permanent position where they will be more than happy to make $85K/year. I have no problem with physicians making a million dollars a year if that is what the market dictates, but using the difficulty of school an number of years it takes is not much of a justification in of itself.

I use my mid levels to compliment my skills. I hate tedious bullshite. So, I have them do diabetic foot checks and care, update PMFSH and med lists, simple med checks, head colds etc. No reason we can't all get along and profit while providing the pt with better overall care.
I started up right out of residency, but I had a unique situation. Many small towns/hospitals will help fund your start. Start small then add ancillaries and mid levels as your practice grows. Hit up auctions of closed practices for really good deals on office equipment and furniture. I had capital saved from moonlighting while doing residency. That's what great about FP residency is the ability to make substantial income on the side in the ER. As an FP, you'll do way better in smaller areas. And the overhead will be much lower.

Odd...I know many and can't point to a single one that has gone 4-5 days sleepless while busting arse continuously while dealing with life or death situations. REAL situations...not theoretical bullshite. We get hazed like mf'ers in school and residency. NOBODY would do that shite hoping for a meager existence.
