If they don't ask for help? Never.
I think the only time I intervened in someone's workout was when some brochacho was doing less than quarter squats with 405 on the bar; like he would just take bar off the uprights, stand for a split second and then put the bar back. Its his life, whatevs.
But, when he was taking the plates off, He was removing all the plates from one side only. "Hey, Brotein Shake," I yelled from across the gym. "You may want to remove some of the plates from the other side, or you're gonna have a bad time."
"Alright bro," he replied. He then proceeded to remove the last of the plates from the same side.
As I predicted, the weighted side was pulled down by that bastard mean ol' Mr. Gravity, the barbell was slingshotted across the gym like a javelin and almost killed some cardio bunnies watching Oprah on the elipticals.
I never saw that guy at the gym again.

I always see guys overpull when doing wide grip lat pulls, wanting to hit the bar to their chest or to the back of the neck! I don't say anything unless I know them well. Never see the ladies doing that.

Still have people do it to me this day. I arse to grass squat and you have 1/4 squat clowns tell me I am going too low and will hurt my knees.
Or the clown a couple weeks ago who told me not to touch my chest when benching
And my favorite the one who told me to drive and explode with my legs while doing over head press
All in the last 2 months

Only done it once.
Kid was in the gym religiously.
saw him trying really hard to get a new PR on deadlift.
Gave him 2 quick pointers and then everyone around cheered when he got that 225.
