Croce’s pilot had a heart attack because someone tipped off the PD of the drugs in Croce’s hotel room so they rolled out right after the show and made the fat pilot run 5 miles to the airport ahead of the scheduled time.

The story I heard was that the pilot was behind schedule because he had been the one partying and thus had to run to the airport. Has it ever been confirmed from a reliable source that he actually had heart failure?
I know there was a crash in the UK where the captain had an argument before a flight and it was believed he had heart failure and retracted the droops too soon.

What does landing thresholds and a rough ride have to do with lining up on the wrong runway?
It is now part of most companies' CRM to verify runway prior to departure. Some aircraft even have a voice alert that states the runway you are approaching and on.

The First Officer was the only survivor and acting PIC. Talk about survivor's guilt....

They had a gruesome one about 10 years ago that’s interesting to read about but hard to watch.
The Captain was the one who lined up the plane on the wrong runway before handing the take-off to the First Officer. So, it wasn't the FO's fault alone.
But, both of them recognized the oddity of no runway lights. Gut wrenching to watch that kind of thing happen.
