Carribean MD school, failed USLME 2x. I doubt her GPA puts her anywhere close to the top of PA/NP school applicants. She'd have trouble getting into PA school in the first place.
What's she going to say at her PA school interview when they ask her why she gave up on medical school? "Well I'm a Carribean med student that failed the USLME twice so I figure I'd try this PA thing out." Can't imagine that going over well with admission interviewers.

at least she is realistic.
time for her to get over wanting to be somewhere in particular for residency and apply deep into 3rd tier programs.
she should pay someone to mentor her and do a serious prep for the re-take of step one and for step 2. She is going to need to be balanced and resourceful at a time when most people would be chewing off their fingernails.
prayer and affirmative statements from you. abundance. there is plenty for us. I am always going to be supportive.
the emotional component is now as big as getting mentoring.

Depending on the field, she'll have the option to apply for fellowships and, AFIK, they wont ask for step one scores for fellowships.

I know a guy who is not the brightest crayon in the box and he graduated from PA school. He had to go to Alabama to get into one but he graduated. I can guarantee he wouldn't be able to pass this step exam. His brother posts here a lot so if he's reading this, don't tell him about the crayon thing haha.

FRom the Carribean it will be pretty hard to get anything outside FP/IM/peds

There is no step 4. Usually it's just boards after that.

Surgery is the only one I've heard more strenuous than OBGYN. She was also chief this past year.
My wife would have done a Gyn-Onc fellowship if I'd allowed. Told her I was done moving and shed have to go by herself. And I was keeping the baby. So she is joining a private practice instead.
She's an amazing surgeon tho. Won some awards with her residency for it.

Also, IMHO, Step 1 was the most difficult exam I've ever taken.
Step 2 and 3 were considerably easier. Even my board was easier.
So if she can manage to clear this hurdle, she might just make it.

I hear you. My wife did her fellowship in Boston while I stayed in NYC. Like a dumbass, I knock her up as soon as she moves to Boston. I was fortunate that my job let me work part time from home, which allowed me to split time between Boston and NYC for a year. She had 3 months left of the fellowship after she delivered. Surgeons are nuts. She only took two weeks off after delivering because she was ready to finish the fellowship.

used to be, but other specialties have poached all the good (and high$) procedures in many places. Cardiologists, vascular surgeons, ortho etc all have first shot at the patients, so its theirs for the taking. There are neurologists that go do brief fellowships and come back calling themselves "neuro-interventionalist", and take all the cases the radiologist with 7 years of training was doing.
Its not like this everywhere, but IR guys now are frequently stuck doing messy or difficult or low paying procedures all day. (vascular access, placing lines, declotting grafts (although kidney docs are poaching even that now), draining pus, ct guided biopsies that were too hard or risky for the pulmonologist etc etc etc.)
Its depressing to see what this innovative and exciting specialty has devolved into.
