They have to give you an internet facing ip address from arin. It's not free to them either. It's like $10 per year for most domains though.

The majority of it, I believe, goes to the company that operates the top-level domain (TLD) that you go with (VeriSign operates .com, for example). Maybe a buck or two goes to the registrar you use (godaddy, namecheap, etc., there are hundreds). A very small portion (like 18 cents) goes to ICANN, the non-profit org that oversees domain names and IP addresses.
There are auctions to determine which companies operate which TLDs, and each country also operates their own TLDs (.us, .uk, etc), sometimes in cooperation with private companies.
