For kids of my generation, being chased by gorillas on horseback was a common nightmare after watching PotA.

I'm not sure the first Ninja Turtle movie is technically a kids movie.
They did a pretty good job of bridging the gap between the cartoon and the source material in terms of how it felt.

It was a more mature, but kids were definitely the target audience

Switchblade Sam freaked me out as a kid in the Dennis The Menace movie. Now as I look back I think dang Christopher Lloyd nailed that role lol.

Those life draining puppets in The Dark Crystal were not child friendly.

Hopper from a bugs life is a great Villain and fits your description.

In the literal sense, very few of the villains mentioned fit the OP's descriptor "not cartoony".

Bob Ewell was scary mean looking in To Kill a Mockingbird.

Movie Shredder was the original Albert Wesker. Had already created and tamed BOWs and successfully used em without tearing down a city and at a fraction of the costs.

My dad was a doctor who made housecalls and would sometimes leave me locked in his car when he called in bad neighborhoods.
That Ewell scene was all too real for me.
And on the new Stargirl series, they had a random school hallway scene where a background actor walked by in that Ham costume.
