Small background moments in films that are quite disturbing.

For example...I have seen Clerks 100 or more times since I first saw it in 96. I was always too busy laughing or connecting to the struggle of wills between Dante and Randall. The last couple times I have watched it, I have noticed one really disturbing scene in the background of the main story that is undolding.
After Kaitlyn accidentally has sex with the dead man in the restroom, the paramedics place the deadman into the ambulance. He is lying in the back and still has a huge erection. Then they have Kaitlyn get into the same ambulance, squatting directly in front of the dead man and his raging bonner. Could you imagine this happening in real life?
Holy shite! This is disturbing....yet still funny.
I honestly have no idea how I never noticed how disturbing this scene/situation was until recently. What other examples do you guys have of small disturbing background scenes in movies?

There’s a scene in Spotlight where Rachel McAdams’ character goes to a sports bar to interview a victim of childhood sexual abuse by a priest, and in the background on TV, a Penn State game was playing.
I noticed it on my own when I watched, and thought it was put in on purpose to be disturbing in that the subject matter was pretty much the same, but the directors said it was 100% an accident and unintentional.

When IT is gnawing on the baby arm while the black kid is getting beat up by Henry. It’s on the screen for no more than 2 seconds but is by far the most disturbing part of that film.
