I've never read the book, but the movie gives me the impression that he was just severely fricking delusional and killed no one.
I know that Ellis elaborates on everything with a ridiculous amount of detail in the novel, so I plan on picking it up soon.

I hated the movie until Bale's monologue on the phone. That was exceptional acting. I know he was trying to be cheesy and over the top in the beginning but I couldn't stand it. Anyway, after all the hype I heard for this movie, I deem it overrated. Bale showed brilliance in some scenes, and mediocrity in others. Blast away at my opinion, but American Psycho = overrated.
