I never thought a QB should be penalized for an INT as a result of the WR tipping the pass to the defender.

Teams do keep track of that.

There was one season where Ben was sacked over 50 times, but around 30 of those sacks happened more than three seconds after the snap.

For a sack, only the team can truly evaluate the players because they know what play was called and what everyone was supposed to do. There might be an OL who just stood there, but it's because he was responsible for a specific defender who peeled back. Or you might have a play where the offense took too long to line up and a safety called an audible and took of with the play clock at 1, and there weren't enough defenders to block everybody.
