For the first 6 months you're going to want in-person visits with blood samples being taken to ensure that your levels are within reason. You will also need to be prescribed an estrogen blocker like Anastrazole.

32 yo and my levels have been dropping steadily for the past couple years. At 180 currently. Would you guys recommend TRT? I also have a gene mutation that predisposes me to prostate cancer at an early age (BRCA2) - wondering if TRT could further complicate that I’m beyond exhausted and weak 24/7

First off, you need to go see a highly regarded endo or urologist.

2nd are you done having kids? If not hold off and talk to the doctor about restarting your natural production with hcg, a serm, and high. if so....talk to the doctor about trt.

You will need to monitor and keep levels in lower normal range.

I have recently considered going to a doctor to get my levels checked. Im 42 and since turning 40 my energy levels have greatly reduced.
The thing that is holding me back is the increased risk for heart attack and stroke. Also the increased risk of prostate cancer concerns me. Any advice would be appreciated!

These are not concerns if you keep your levels in range for your age. Even if you just keep them in range at all really.
