Intercostal Nerve Pain

I have been having pain in my back for several years now. It starts near the spine and passes around my ribs. I'm fairly certain that it is intercostal neuralgia. My PC seems clueless. I had an MRI and nothing was found, but the pain is there nonetheless. The symptoms are consistent with IN.

Two questions...
Anyone have any experience with this?
What type of doctor do I see? Is this spine, or straight to pain management?

If you can pinpoint spot near your spine, see if neurologist can inject the area around the pain with lidocaine, almost like a trigger point injection. See if you get any relief from that. Granted, it won't be as good as a direct intrathecal injection, but it might help point to where the problem really is.

I'm fresh out of ideas otherwise.

It could be post herpetic neuralgia from shingles. Did you have a rash on the same side as your pain?
