Make sure the thermometer is reading correctly. Do this by removing it and check it to boiling water, should read 212. If not, adjust it by holding the nut on the back and turning it either way. Repeat until thermometer is reading 212.
You should calibrate your thermometer every so often.

some of these suggestions seem waay too prescriptive. you shouldn't need to stack your charcoal in order by size, etc... i literally dump a couple pounds in, make a little "hole" in the middle, toss in a fire starter cube, and cover it up. Let it burn for about 10 minutes then close the lid with the bottom and top wide open until it gets to the temp you want/need then start choking your vents down to maintain temp.

How long are you waiting?
If I'm trying to get mine screaming hot to grill some steaks or something, I usually use an ash tool to knock off any ash from the lump charcoal from my last cook, fill with charcoal as needed, throw a lighter stick in there and leave lid open with bottom vent wide open until lit and up to temp. It usually takes about 25-30 minutes to get up to 600-700 degrees.
You can speed this up a bit if you have a portable fan that you can angle at your bottom vent.

Seems if I just put a starter in one spot, it takes a while to spread evenly to cook

Get a clothes hanger and bend it so that it can go into all of the holes from the bottom vents. You’ll have lot of ash fall down. Remove the ash and you should be good
