At what time do you flip the sack to get the ones on top cooked?

enjoy the shits eating crawfish boiled in swamp water, mud, and some bits of leftover rotten bait that is in every sack

Growing up my dad had a huge pot used by military cooks to cook for troops. That is what we boiled crawfish in so we did not have a basket. We would remove the crawfish from the sack and clean them . Then we would return them to the sack and boil them using the sack as a basket.

Seems like that would make for uneven cooking as the ones in the middle would not come up to temp as quickly as ones on the outside?

This was about 25 years ago, but I think we did it in 2 batches. It was definitely not a full sack.

Ya, I know a couple places that wash the crawfish before selling. I've seen the rig at one place, it's pretty cool.
