I won't talk about ambulances because those are always going to be high costs.
Let's start here. When you buy a tv, do you just walk into the store and pay for it with no questions asked?

That in itself is the single biggest reason it was destined to fail.
It's like forcing an car insurance company to give you coverage and pay for damages after you just wrecked your car.

It's not complicated. Obamacare was seen as a solution to a healthcare problem in the United States The problem was that health care cost had risen over three decades because demand had outpaced supply of medical care and facilities. This was largely due to med schools restricting admissions, and we could argue over why they adopted that practice.
The solution was to increase the supply of medicine. Economic theory and empirical data over 200 years supported this obvious approach.
Obama care did the opposite. It added a layer of clerical and administrative costs while doing nothing to increase the supply of medical care and facilities. No additional physicians, no added hospital beds, no added emergency rooms. It reduced the number of hours per day of physician availability for patient treatment because of the documentation requirements. New refgs required that physicians documented for the clerical overseers what they had been doing all along and continued to do. One family practitioner who is a friend told me recently that instead of being able to leave his office at 5pm he was there until 10 every evening doing non productive work. A lady who is part of his medical group has a large practice, and to free her for patient care had to hire five additional clerical assistants to keep up. The quality of care declines as clerical errors on recording test results lead to erroneous interpretations.
This bill is beyond a failure. The stupidity of those who designed it is just incredible. Economically it does the very opposite of what was intended. Less care available at higher cost.

Personally, I do my research beforehand so I know exactly what I mean to purchase. So to answer your question: yes.

If people were choosing to pay the tax rather than buying the insurance, it failed at what it was intended to do. All those people still used the services of health care and still didn't have to pay for it.
