Easy one...that's the infamous Boeckeler Instrument, everyone knows what that used for. Hell, I have three of them myself and use them when I need to do a little nano research.

It is the biological specimen holder for an ultra microtome. It is used to cut thin sections of your liver while they stand around talking about you in surgery.
They cut thin slices of the section. stain it, look at it under a microscope and decide whether to take out your liver if they see cancer, or leave it in if not.
Regardless of the outcome, they make fun of you while waiting, because that is what folks in surgery do to kill time.
