We are looking to regrade it and remove a lot of dirt. The patio is poorly designed and the lowest sitting point of the backyard. It was never something that bothered me.

I would go buy a drain cover and box, go stick it in center of yard and tell them they got drainage. When they realize it doesn’t work correctly, argue with them a bit longer and it will be July before you know it.

Tell them that 50% of the world's population lives within 3km of water. They are lucky to be one of them.

I would probably take a different approach.
I wouldn’t completely change the grade. Too expensive for an investment property. I would fix any standing water issues. That is easy with French Drains. After a heavy rain, mark locations of standing water, buy 100ft rolls of corrugated drain pipe and a few drains. This will cost a few hundred.
Tell your tenant your willing to fix it by purchasing the materials if they will help provide labor to dig the trenches to the street.
You fix a problem for a few hundred bucks and improve your property, they get no standing water.
Don’t communicate with the parents. Just the person who is on the lease.
The health concerns with standing water would be related to mosquitoes and the associated viruses.

Are French drains cheaper than regrading? Getting very inconsistent info on that.

Op, as said don't communicate with the parents. Only the tenant.
2ndly, if you don't think its a huge issue don't let it be.
First thing I'd probably do, is go get a lawyers's opinion and maybe have them write a quick note to the tenant on how a swamp area is not a "health" issue lol. So half of Louisiana is a health issue? That's absurd.
How easy is this to re rent? I'd strongly consider offering them to break the lease and move on. Maybe mention that, and that will shut their arse up if the rest of the house and location is good.
If you offer them to break the lease in 60 days, and they refuse then that gives you a degree of legal protection against any other "health issues" or liability as they are choosing to stay.
