What’s a lab like that gross a year? She likely was making damn good money already right? $400,000 or something? I’m guessing she didn’t make this over a handful of transactions, but more like 200,000 transactions. It’s not like she was hurting for money .

Depends on several factors. What type of specimens is she processing? How many of those is she sending out to other labs? How many tests are the providers ordering on said samples? Are they medically necessary (this impacts reimbursement)? Federal or private payers?

Very few boutique lab owners are successful. They aren’t cheap to operate and getting providers to trust your lab with their samples vs the well know big guys (Quest, LabCorp) is a tall task. Then you need significant volume to break even. Thus her relying on kickbacks to her providers. Sadly it’s more commonplace than you may think.
ETA: You also need a damn good billing department/third party biller, as well. Those are hard to come by.
