Well of the top of my head.. In the 50s, a botched polio vaccine that was administered paralyzed and killed children.

Look up "cutter vaccine"

I'm sure that's not the only instance

Three comments here

1) that was literally at the very beginning of vaccine science.

2) they don't use that live virus method any more. They use inert material that tricks the body into making antibodies

3) with computer modeling, they can develop safer, faster, more effective treatments. You're talking about how the Wright Bros crashed, and therefore you don't feel safe getting on a modern jetliner.

Vaccines are safe and effective. And this one will be too. Final answer.
