Doctors and RN salary is generally higher than server and cook salary, so your markup is higher.
Well that is part of it. The other part is that no matter what the price, there will be a large segment that doesn't pay it. So the cost gets passed on (via markup) to others. Insured patients, btw, don't pay that. The insurance companies negotiate a separate price for procedures on their clients. That said, there is still a mark up because of the write-offs and the below cost reimbursement from government programs (though that is hit or miss. Generally Medicare is OK while Medicaid pays less. There are exceptions).
The people REALLY taking it in the shorts are people that make too much for government programs but don't have insurance and have to pay cash. Ouch. Get insurance kids.
