I think a lot of it is injuries and strains that happened in high school that weren’t properly treated. Then they finally get around coaches that give a shite about their future and the issues are dealt with instead of riding them into the ground.

I think this is part of the issue. The thing is, there are a million things that could adversely impact a pitcher's throwing arm when it comes to rest, mechanics, strength and conditioning, pitch counts, innings, etc. No one, from the pros on down to youth league, has a firm grasp on any specific one of them.
I think that using technology like the Motus sleeve to measure arm stress and customizing throwing programs to each individual can help mitigate or delay injuries. There also needs to be customization of the strength and conditioning because just building power in someone's legs without balancing it out in their upper body is a recipe for pushing too much force through their elbow. The muscles around each joint need to be strengthened.
