I've seen hitches, receivers, rear bumpers, and front portions of frames ripped off. I've even seen frames bend and rear/front ends ripped out. I've never seen a ball break, and I've been involved in lots of really quick and large shock loads during recovery.
I've always carried a purpose-built recovery strap and d-ring shackles. Most people just use chains. I don't like being around in that situation, but a lot of times, they don't have the option of using something designed for the purpose of yanking.
Eta: When I'm advising, I don't ever let anyone I'm around just use the ball as the primary anchor point. The ball basically holds the chain/strap in place, and the main yanking force gets transferred to the ball mount, then the hitch
