I assume playing the games helps, especially in understanding the nature of the monsters (though the janitor and those nurse creatures were pretty self-explanatory.) I never played them myself, so I have no clue about some of the monsters. The plot itself is laid out pretty well in the movie. Town run by some apocalyptic witch-hunting cult finds out that one of the children (seemingly fatherless, iirc) has psychic and/or magic powers. They burn her alive as a witch, but she's too powerful to actually kill. Instead, in immense pain, she lashes out with her psychic/magical powers, creating an alternate-world hellscape out of the entire town, with the only refuge being in the church. She sends out the last part of her that isn't filled with rage and pain in the form of Sharon (apparently she has the power to do such things) Sharon is adopted but still has a subconscious connection to Alyssa and Silent Hill yadda yadda (I assume how they got there isn't confusing.) Since it's an alternate world, Sean Bean and the cop, who are in the 'real' world, never encounter the inhabitants of the alternate world, including (now) his wife and adopted daughter. And so on. Alessa slaughters everyone except Sharon(who now seems to be Alessa as well), the mom, and her own mom. Then, even when they leave Silent Hill, they still don't leave the alternate world.
Yeah, writing that all out, from 3-4 year old memory no less, I can see how it might get confusing.

To add to that, they never get out of the alternate world because Sharon opened her eyes at the ends and saw Alessa if I recall correctly. Her mother tells her not to look but she does anyway. I think that has something to do with it.
But yeah, very strange to people who have no prior knowledge of it lol
