How could it be that none of those "fed" pictured have friends or relatives that they know are part of the "not fed"? At some point, wouldn't you think they would come to collectively realize that they are actually in a position to do something about it?

Starvation, abuse of any sort actually, can break down social bonds. I'm sure the NOKO military and other security personnell are the most cold-blooded picks from that country's 25 million people.

That collective realization typically happens through some kind of communication process (media, face-to-face networks) that builds solidarity or some sense of collective interest. If you control those communication processes, you preclude the organization of potentially revolutionary networks.
