Got denied by a foreigner the first time. Trying take two. He said that they aren’t offering a customer loyalty discount at this time.
Was that the loyalty department? I dialed 1-800-DIRECTV, said "Yes" when they asked if I was a current customer. When it asked why I was calling, I said, "customer loyalty department". The automated system then asked me was it to change my current service or cancel. I said, "Cancel". It asked if it was because of a move, and I said, "No". I was immediately transferred to the loyalty department, and the guy I spoke to was clearly American.
ETA - it isn't called a loyalty discount. I just said I heard they were offering a promotion, and I would like to see if we could lower my bill. Sam called it something like a "buy-in promotion", but I didn't have to buy anything.

Call 3 went a little better for me I think. Was told every account was different, blah blah blah. Ultimately I upgraded my package from Xtra to Ultimate but at a promotional rate of around $53 cheaper for 12 months. I already have HBO and Starz, she said Starz is included in the Ultimate package.
40 minute phone call, and I feel like i've been ran through dry. Hopefully it was worth it. Congrats to those that had an easy phone call.

Exactly how I did it.
