Tony as a mafia boss was just very unrealistic. Bosses don't murder people like he did. That task is for soldiers and wannabes. He also makes himself too easy of a target. Just strolls down the driveway in his robe to get the paper every morning, drives all the time without a bodyguard, etc. Seeing a shrink, particularly a woman, wouldn't happen.
I loved the show but no mob boss would survive very long acting like him

I'm not disagreeing, but it is jersey.
He's not a real mob boss.
ETA or rather, he's no a boss of a real family

Vincent “Chin” Gigante, boss of the Genovese crime family, who was noted for walking the streets of the Village near his Sullivan St. social club dressed in pajamas and bathrobe and mumbling to himself, died in a federal prison hospital in Missouri on Dec. 19 at the age of 77.

The shrink thing is just a dramatic conceit of the show that allows for a different perspective on the traditional mob boss. It allows for a peak inside the mind of someone in Tony's position that as far as I know hasn't ever been shown in that same way. You get to see an exposition of his motivations, his thoughts on family, his upbringing and how he became how he is, etc. It was used to ask questions like if he was capable of change. And more. It isn't something that I would use to criticize the show for not being realistic. It's kind of used as a soliloquy as a device. It is deliberately ironic, and the show even makes fun of its ridiculousness.
