Every state didn't expand Medicaid, and the penalty for not having insurance isn't particularly harsh (e.g. only comes out of a tax refund).

There won't be a transition, because nobody knows what Republicans want to transition TO. Given that uncertainty, there's no reason for anyone to think the current system (ACA / exchanges) will endure, so why waste time/effort/money into entering them?
There's no legal way they can revoke health insurance from those who have already purchased policies for 2017, so that's not what we're talking about.

I know Tom Price has had a bill in the House for the last 4-5 years, several bills were passed by both houses and sent to barry only to be vetoed.
Trump has had his plan on his website for a while.
The Congress, both GOP and Dems, have been trying overturn a law passed way back in the 1940s, the mccarran ferguson act of 1945.
Its basically given a huge anti-trust exemption to the big boys.+
This has been a huge push by Trump and the GOP.
Get rid of that, and the competition opens up

None of those bills were serious attempts at health reform that would lower costs for any Americans (except perhaps the rich, by repealing taxes) - all they were were straight-up repeals of the ACA with no thought to the consequences.
