I have it through Nationwide. Runs about $70/mo for wellness and medical. My dog has torn CCL in both rear legs and already had one fixed to the tune of $1500+ and the insurance reimbursed me 90%. The insurance will not pay for anything pre existing or hip displacement. Your vet should have some info on this.
