She'll have limited options, either a less competitive program in a more populous town or more options in less desirable towns, BUT it's just residency and she wont be tied down wherever she does the residency. Depending on the field, she'll have the option to apply for fellowships and, AFIK, they wont ask for step one scores for fellowships.

Really the road is just more difficult but not impossible. Most patients won't care where she did her residency, they'll just care if she treats patients well.

ETA: Second time. Yeah. THAT could be trouble. I wouldnt discourage from continuing though.

Backup question, since she went to med school can she just go be a nurse or start with nurse clinicals or whatever it is they do, or would she have to go to nursing school.

They ask for USMLE scores for fellowship applications.
