I'm at a tertiary center and our number of tPA transfers have gone way up in the past year or so. Most given at rural-ish EDs where they don't really know what they're doing and the complication rate is much higher than at a proper stroke center. I wish they'd CT and GTFO (even the CT is probably not necessary as we usually repeat it).

There has definitely been a shift to include all stroke patients in the tPA window. Saw a dude last year with facial TINGLING for an hour. Not droop? No deficits. They were considering tPA. Lunacy. He was hypertensive as well with a headache working for days.

This is a dude walking talking and drove himself there? His CT is negative, but for all you know he is getting Bells Palsy? Go from walking to brain bleed or GI bleed.
