I was playing my ps4 and everything was fine, but when I went to put it into rest mode to go to bed, it randomly ejected the disk (GTA V). Odd, thought I might've accidentally bumped the button so turned it on to insert the disk, the controler blinked 5-10 times then eventually came on. System won't take the disk and makes a grinding sound
When you attempt to put it in.
UPDATE: Seems to have fixed itself, woke up and everything was ok. That was really bizarre

I had the same issue. After much searching online, I found a few "fixes," none of which worked for me but they might for you.

1. The middle rubber foot is pushing against the eject button. I can be pulled out (and put back in) without cutting.

2. Start in safe mode and select Rebuild Database. Don't worry, this does not wipe any of your data (a very real fear I had with all my downloads and lack of wanting to re-download).

3. Was told by the tech support folks to shutdown for a few minutes and try using it standing on its side (no reason was given that this might do anything).

4. There was also talk of turning the disc ejection screw because it was too tight/loose. Not the easiest to find and you will need a precision screwdriver (think eyeglasses type).

I ended up having to send it in for repair. To their credit, I was out of the 1 year warranty but they still fixed it for free.

Mine has done that before but it always resolved itself after restarting it a few times.
