If you want to change baseball's divisions in a rational way you probably have to increase the total number of teams by two to get to 32 total. Then you have the potential for four divisions in both leagues.
Let's arbitrarily make the case that Sacramento and Austin are your expansion sites (large states with well supported baseball attendance, affluence, etc) because San Jose is SF territory, Las Vegas brings gambling and the other probable sites like Montreal, Providence, Charlotte, and Memphis don't look like they would have the combination of affluence and population to support a 162 game season.
Then you could talk about moving around a few teams while keeping intact a few blocks like:
yankees, red sox, orioles
cardinals, cubs, reds +/- pirates
phillies, mets
dodgers, giants, padres
tigers, twins, white sox, indians
A's, mariners

This is really the best answer. Make sure there at 16 teams in each league so you don't have a daily inter league matchup.
I could see 4 4-team divisions with maybe 2 wild cards....but I also like the idea of two eight team divisions with each champ gets a "bye" and then four wild card teams play a series.
