I'm sure you are familiar with the concept of a complete and accurate tax return. Well the same concept applies to other filings with the government. If you do not provide complete and accurate information when registering to vote, then you shouldn't expect to be allowed to vote until you provide complete and accurate information. It is the applicant's responsibility to provide complete and accurate information, not the government's responsibility to gather said information.

Where in the article did it say the "lost" applications were incomplete or inaccurate? That seems like your assumption, which I guess is fine since the State has refused to comment on it.

Applications get lost. Things happen, but 40,000-50,000, and all from a particular registration drive? That's fishy.
