I don't know a damn thing about Ultron, but I think it is in the slight realm of possibility that Tony Stark and Captian America come to blows over the creation of Ultron at the beginning of the movie and then Ultron kills CP towards the end.
I guess you would have to set up Bucky taking over as CP prior to CA3 though
