If they were able bodied and could work, I would tell them to go get a job with insurance. And, for these purposes, no job with insurance is beneath them.

If they aren't able bodied, I would require them to pay what they could, and have some sort of separate pool for them -- somewhat like Medicaid.

One of the largest contributors to spiraling healthcare costs and insurance costs was tying insurance to employment. fricked things up beginning in the late sixties or so and spiraled out of control after that.
