This might be impossible. At the very least, you will probably have to create the fields and put the formulas in Acrobat manually.

Something to consider: would a web page work for your purposes?

A webpage would likely work just as well, as long as I can manage a secure log-in. Suggestions?

I'm with Korkstand. I've created a few forms in Acrobat and don't think anything from Excel transfers over directly. I haven't tried to turn any into a form directly from Excel though.

I think it'd be easier to restrict and control the excel form, or manually input the form in Acrobat.
