My son is 29 years old, tall and slim. He had both decompression with a PT and the injections. Both only offered temporary relief.

He has a problem with severe pain when he stands for more than a couple of minutes. He takes pain medication or suffers.

I was going to say these 2 options but looks like he already has done both How long has he done the decompression and is it with a Chiro?

Might be a candidate for the knife.

Dr. Munshi did my back surgery in Lafayette. Was my second surgery, did a 2 level fusion with 8 screws and 2 rods. Could not walk hardly at all before the surgery. Now I'm back to fishing and playing golf. Biggest thing I can say is to keep your weight in check... stay where the Dr. recommends you keep your weight.

Go see Dr. Henry Eiserloh, ortho in B.R. I have seen him have success with complicated cases. Baton Rouge has better orthos and neuros than Lafayette. I do not know that I would treat for a back in Lafayette when B.R. is so close.
