I would coot that son of a bitch.
J/k Seriously, has he ever bitten you? I put my lab on ivomec after three years of heart guard and didn't have problems. He never had worms though. Sounds like a neurotic disorder. Maybe try a different vet's opinion.

Hit him harder
The first bite is a beat down, the 2nd one is a bullet

Don't just put him down, that's absurd. Have a vet look at him

I would say the only thing possible that is not listed is somebody messing with him when you can't see. I'm not sure of your situation but people can mess up dogs by hitting kicking bashing the fence etc.
Had a similiar situation to this with a blue heeler I had. Had never been aggressive then out of nowhere he tried to bite my FIL. Sent him to a new home, behavior problems dissapear. We suspect the neighbor, who had a history of teasing and doing mean things to dogs, was screwing with him.

Easy fix. Everytime he goes to growling or biting, throw him on his side and keep him pinned down with you hand on his neck right behind his ear. Keep him there till he calms down. A few times of doing that and he will quit. It's just a dominance thing. If he has a toy that he gets possible over continue to take it away like it is yours. He will get the hint and stop. It's all about patience. Some dogs take longer to get it but in the end your dog will understand his place in the pack.
