PS.. i wont hold you to the name of this dish, just as long as it has similar ingredients.

Donald Link has a solid recipe in his cookbook. i usually don't care for this type of casserole but his is pretty good. i don't have the book handy to give the recipe but if you have access to the book or maybe it has been posted on the web.

River Roads One or Two has a very good one. It calls for the garlic cheese if you can find it. I just add garlic. I also add mushrooms and chopped waterchestnuts for crunch.
