I don’t know that I particularly like the way NYC does this, but it’s major and minor party noms at the top. Independents at the bottom. If multiple parties nominate you, you’re on there multiple times.
Sliwa would have been on there three times if he’d accepted the conservative party nomination.

Feels like he should have to split his vote and not just add up all his votes from 2 different parties. Why don’t candidates just form a bunch of random parties and get their names printed 10 times on the same ballot??

Yep, that's exactly what I was thinking. It's a stupid way to have a ballot and makes no sense. With all the space they are taking up, they could easily just have the candidate name once and then list all the so called parties they belong to.

So to some extent that’s kind of what happened here. Sliwa formed a party around an issue important to him with the “Protect Animals” ballot line.
He’s a strange fellow, but I love him for that. What’s going on with cats and dogs in shelters around the country right now is appalling.

FWIW, the first 4 lines A B C D are parties that have been around. And you can look down and see the Public Advocate, etc is organized on the same lines.
It’s after that you have new parties.
As I think about it, I can see a justification for giving historical parties ballot lines, even if they’re nominating a candidate already on the ballot. And they set up the ballot on lines rather than by candidate, listing all the parties nominating them.
Looks strange to my eyes but New Yorkers probably get it.
