I've always interpreted this as meaning that you intended to do things, but never follow through.

"I meant to get my engine checked before the road trip. Now I am sitting on the side of the road."

this is incorrect. if you never follow through, no roads would be paved. it means you do something you think will be good, but it ends up being a net negative happening because of your action.
