It’s usually fatigue. It can be loss of concentration, or aiming the ball, rather than normal movement.

Yeah, that is the effect of fatigue on pitchers. For fielders, it usually ends up causing a lack of concentration, rushing throws, and also aiming the throws The first leads to poor decision-making, and booted or whiffed balls. The latter two results in overthrown, underthrown, or off-target balls to the baseman or cutoff man.
