Hard to know what came first, culture or “crap service”, but it would be insulting in Europe if the waiter was bothering you the whole time.

Europeans spend lots of time socializing during restaurant meals, they normally stay ~2 hours.

Without free refills, there isn’t much for a server do to throughout the meal. (You can get free water if you ask, except in places where the tap water isn’t safe to drink)
