Blood eagle, as previously mentioned.
There was an ancient Persian torture method called "the boats" that is about as bad as anything as I could ever imagine. It supposedly took a couple of weeks to die using that method.

Both are a little too fantastical in my mind to have ever been really used. Especially the boats one. Sounds way too labor intensive. And the blood eagle—seems like the victim would die or pass out too quickly to even get it done as described.
