I can just see how a pre-op appointment would go.

"Well sir, we're going to need to keep you under anesthesia a little longer than normal so your obese doctor can take federally mandated breaks."

You post this in jest, but I read one of Tom Clancy's first Jack Ryan books and nearly this exact scenario was written.

Ryan's wife (think Anne Archer) is a visiting doctor in England while Jack is posted there. She sits in on an eye surgery where the doctors performing it go to lunch and have a pint while the patient is kept under general anesthesia. She nearly has a shite fit complaining to her husband about it.
